Non-Specific Low Back Pain Exercise Therapy

As often discussed in my research reviews, pain in lower back remains an enigma in modern day health care. It is a common, disabling, and costly condition that we know relatively little about in terms or pathoanatomical causes and effective treatments.

The goal of this large meta-analysis was to evaluate the effectiveness of exercise therapy in the treatment of low back pain in adults. Exercise therapy was intentionally defined in this study as a heterogeneous group of interventions from general physical fitness interventions to various types of stretching and aerobic exercise protocols. 61 published reports of completed, randomized controlled trials comparing an exercise intervention to placebo, no treatment or other conservative treatments were included in the analysis. Patients suffering acute ( 12 weeks) low back pain were included in the studies (most of the studies involved chronic patients).

The trials evaluated provided the following insights:

• there is clear evidence that exercise therapy is at least as effective as other conservative therapies for chronic low back pain, despite conflicting evidence that it is any better
• for acute pain in lower back, exercise are not more effective than other conservative therapies (note that exercise therapy is not the same as advice to stay active, which is currently a recommended, evidence-based intervention for low back pain)
• there is some evidence to support a graded-activity exercise program for sub acute low back pain in occupational settings

Conclusions & Practical:

This study concludes that evidence from randomized, controlled trials demonstrates that exercise therapy is effective for reducing pain and improving functional outcomes in patients with chronic low back pain. It appears that clinically important improvements are more likely to be seen in healthcare environments, meaning that exercise therapy may work better when supervised by a healthcare provider (see companion article regarding strategies for implementing exercise therapy for low back pain).

It seems intuitive that exercise would be an appropriate intervention for low back pain, provided that it is administered by qualified professionals in a safe, supervised environment. This seems to be a situation where the literature still needs to catch up to clinical practice. To this end, I feel more research needs to be directed toward the effectiveness of combining physical and clinical interventions commonly used in practice. I feel the absence of clear data on the treatment of low back pain is due to the lack of “clinical reality” of most randomized trials.

Most manual medicine practitioners combine a number of interventions in addition to exercise to treat these patients. In order to studies to reflect the benefit of such treatment regimens, more large-scale trials need to be performed with this type of design.

Neck Pain Exercise Therapy

Neck pain is a common phenomenon. It can be accompanied by headaches, tingling and weakness in the arms, stiffness and shooting pains. Guided movement therapy is thought by many to be the most effective treatment for neck pain.

The following are possible neck pain causes:

-a problem in the cervical spine, such as a misplaced vertebra, disc damage or abnormal curvature of the spine
-muscle strain
-spinal joint arthritis
-pinched nerve exiting the cervical spine

Strictly speaking, these are not root cause of pain; rather, each one of these conditions is caused by something else, such as poor posture, injury or age. Once it is determined whether the pain is related to nerves, joints or muscles, you will be able to determine what action or situation led to it.

Motion is perhaps the most natural and effective form of medicine. Our bodies are designed to be active. When sitting, we use a number of muscles to hold ourselves upright; when done properly, we get a low-grade workout in any position. Movement serves a variety of purposes. It keeps fresh blood flowing to our muscles, flushing out waste products and supplying oxygen to be converted to energy. Movement allows our muscles to stretch, maintaining the elasticity of muscles. The final rehabilitative component of movement is that it helps to keep muscles toned. Strong muscles support structures around them, such as the spine.

This explains why exercise therapy is an effective treatment for most chronic pain conditions. Stretching the neck encourages normal disc space between vertebrae, helping to prevent or correct disc bulging and herniation. Strong muscles allow the body to maintain proper posture, which helps to reduce and correct abnormal curving of the spine. Strong, elastic muscles are also less prone to strain. A study of female office workers showed that strength training neck exercises decreased or eliminated neck pain in 73% of participants.

Is Exercise Therapy a Solution for Back Pain Treatment?

Exercises are considered one of the best way to cure back pain. The best way to cure a disease is to find the cause behind it. The pain might occur anywhere in the spinal cord. There are different regions of spinal cord where the pain can occur like lower back or upper back. The time for which the pain persists may also vary. Therefore the first step is to diagnose and find the particular region where the pain occurs. But the mere fact is that, there is no magical cure for such pain. We can work to reduce the pain and participate normally in day-to-day activities.

Proper footwear can potentially prevent and reduce back pain. Every time your feet hit the ground a reacting shock is experienced by the spinal cord. Any biomechanical imbalance in the process causes lower-back pain. After approximately 500 miles the shoes tend to lose their shock absorbing capacity. So it is necessary to keep a track on how long you have been on these shoes and change it before the soreness begins. Other than the structural misalignments, unequal leg length might also be a reason for the pain. So it is safe to diagnose the pain as soon as you start experiencing it.

The muscles in the back help to support the spine and body. Strengthening these muscles will help treat and prevent back pain. Core exercises strengthen back muscles and abdominal muscles, taking pressure off of the back. This could potentially reduce or eliminate lower-back pain.

In most of the cases lower-back-pain is caused by improper postures. In this case the pain is due to imbalance and not because of weaker back muscles. This results in compensated body movement. The only way to solve the compensation is to re-balance the body by providing proper instructions to the body. We need to show muscles how to engage properly. As the body go back to balance the lower back pain will leave you forever.

There are a variety of therapeutic treatments available to people who suffer from different back injuries. Choosing the correct combination of therapies is the key element of any treatment program. Therapy options will probably include pain relief in the form of pain medications or something more exotic such as acupuncture. Physical therapy and chiropractic care will also be options to consider which can not only relieve pain but also help prevent future injuries. By avoiding activities that aggravate back problems, learning to properly lift heavy objects, using a firm mattress to sleep on and using correct posture, back injuries and pain can become a thing of the past.
